The Jersey City Medical Center's Community Support Services (CSS) is currently seeking qualified candidates for the position of Program Coordinator. The purpose of CSS is to assist adults with serious and persistent mental illness to better manage their illness, increase their mastery of life skills, and improve their sense of well being while coordinating and maintaining their housing needs in a housing first model. This is achieved through personalized, collaborative, and flexible outreach and supportive services. CSS provides the full range of case management and support services including assessment, services planning, services linkage, psycho-education and support, advocacy, crisis intervention assistance, direct assistance in obtaining the basic necessities of daily living and in performing daily living tasks, symptom management, and substance abuse education and intervention. CSS works in collaboration with other programs of the Behavioral Health Department including the JCMC inpatient psychiatric and detoxification units, psychiatric emergency screening center and the adult outpatient and day treatment programs as well as other community providers.
The Coordinator will participate in the following activities: Ensures that program/unit/activities operate as planned and services are delivered in accordance with relevant policies and procedures. Meets productivity assignments. When providing direct services, meets the standards for Senior Primary Therapist in relation to clinical assessment (Comphrensive Rehabilitation Needs Assessment), treatment plan development (Individualized Rehabilitation Plan), treatment plan (IRP) implementation/ monitoring/ updating, chart documentation, termination and follow-up. Participates in crisis intervention when required. Follows up post incident.
Supervision of day- to- day services. Provides documented supervision of assigned staff. Monitors all recovery plans and intervenes when required. Ensures that clinical charts are appropriately documented; ensures chart compliance with relevant Policies and Procedures, insurance and regulatory standards. Evaluates all referrals to the program and conducts clinical assessments to determine eligibility for services.
Makes hiring/termination recommendations to Program Director/Administrator, initiates disciplinary action and evaluation reviews. Assigns/directs staff as indicated. Manages scheduling for the department. Participates in program development, makes recommendations to Program Director / Administrator for program growth, and participates in RFP s and grant writing. Ensures that needed supplies/resources are available for unit operation and prepares P.O. s when necessary. Participates in all department, hospital and outside agency/institution committees as assigned/delegated.
Prepares and submits statistical reports, including: Collects data for monthly QA/PI/UR reports as delegated, ensures that all grant data is gathered and reported to the Director, and gathers customer satisfaction data as assigned.
Master s Degree from an accredited college or university in social work, rehabilitation counseling, or other related behavioral health or counseling program. Must be a practitioner of the healing arts, including but not limited to a LCSW, LPC, LMFT, and LRC. One year relevant experience post Master s Degree. A current driver's license issued in the driver's state of residence is required. The license must be without restrictions and with all necessary endorsements appropriate to the class of vehicle being operated. Current CPR and First Aid Certifications required.
