Northwest Center\u2019s Early Supports Program is hiring for a Hospital-to-Home (H2H) Family Resources Coordinator (FRC)!As a\u00A0Family Resource Coordinator for the Hospital-to-Home team, you will play an\u00A0integral role in supporting babies under 6 months of age at risk for developmental delays and their caregivers in accessing therapeutic services in their homes.\u00A0You will work collaboratively with a multi-disciplinary team on enrolling new families into services, supporting the emotional well-being of caregivers, accessing community resources, and ensuring a smooth transition into Northwest Center\u2019s care.\u00A0This role offers a manageable caseload of around 35 \u2013 40 families. You will be providing a meaningful impact on your community by connecting babies and families with early support services. Develop a basic understanding of infant feeding by breast, bottle, or tube. Clearly communicate with families what to expect during the evaluation process as well as what it will look like to participate in Early Supports. Coordinate the first meeting at the hospital if the family indicates that is their preference. Communicate with the referral source the status of the referral.Caregiver Support Comfort in talking to families about traumatic events and big feelings. Understand how prematurity and/or medical trauma could impact infant development and parenting experience. Have comfort in handling infants and supporting parents with baby gear. Ability to read parent/ caregiver stress cues and be responsive to infant needs. Understand and be comfortable with screening for perinatal mood and anxiety disorders. Make referrals to mental health professionals as needed. Identify resources available in a specific geographic area and community.Program Development and Team Collaboration Ability to work and collaborate on a team. Ability to reflect and exhibit a growth mindset. Demonstrate a desire to learn, particularly self-directed learning (i.e. independent research, seek out relevant training opportunities, etc.) Actively participate in Best Starts for Kids Training. Help present cases and problems of practice at coaching meetings. Take on special projects as appropriate.Requirements: AA Required,\u00A0BA Preferred Prior experience working within different data systems preferred Knowledgeable of the Family Resources Coordinator function, processes and paperwork\u00A0and\u00A0willingness to be flexible to best meet the families where they are at Early Supports experience\u00A0preferred Strong interpersonal skills to include but are not limited to a positive attitude, strong listening skills, and problem-solving skills Relevant education or experience in case management, social work, social welfare, family studies, infant mental health, early childhood development, or special education Must have or obtain current CPR and First Aid certifications Valid WA driver's license and personal mode of transportation Candidates with experience providing services in a non-English language experience\u00A0desiredNorthwest Center is committed to offering reasonable accommodations to applicants with disabilities.
